- Capital Health Plan (CHP) Principal Choice: This coverage is available only if you live or work in Calhoun, Franklin, Gadsden, Jefferson, Leon, Liberty or Wakulla counties.
- Florida Blue HMO BlueCare 56: This coverage is only available if you live or work in the Active BlueCare HMO service area. You may not select this coverage if you live or work in CHP eligible counties.
- Florida Blue PPO BlueOptions 03769: This coverage is only available to employees who live and work outside the CHP and BlueCare HMO area

Flex Dollars
FBMC contributes flex dollars to full-time and part-time employees opting-out of medical with proof of coverage. The contribution amount is based on the employee’s employment status.
Wellness Rewards
FBMC provides a quarterly wellness reward to Full-time and Part-time employees electing to participate in WellSteps. Only employees who reach Gold or Platinum Wellness levels at the end of each Wellness Activity Quarter will receive the reward for that quarter. Reward amounts are based on the employee’s healthcare coverage level. Employees opting-out of medical with proof of coverage will receive the same amount as employees with employee only healthcare coverage.
